I really liked the Washington Model for Screening for Bias.  I would add identifiers to it to include being careful/respectful about students experiencing homelessness issues, mental health awareness, and formerly incarcerated students.  I particularly liked the 2, 1, 0 grading system and then also the space for comments and more detailed information is available.

I gravitated toward the BC Campus Accessibility checklist. It provides basic accessibility suggestions for working with OER. I think it is a good starting point. With additional updating to include tools that can assist with incorporating accessibility during the development process. Tools such as Blackboard Ally, Pope Tech, WebAim Color Contrast checker and more.
