Online Communities of Practice for Teachers

I would like to find out how we can coordinate and facilitate online communities of practice amongst teachers. Of interest to me are the online tools and features that we can utilize as well as tools to manage and monitor these activities. I am aware that the Philippines have introduced these based on a report they presented in Kigali.
Kindly assist if you have an idea or resources on how we can go about it.
